Anyone tell me the correct way the lever goes on the stud ?

My guess is spring first, then lever through the large beveled hole, then washer and nut ?

The bolt through the main tube locking ring should be at the spark plug end ?

The end of the operating rod should ride over the locking ring to keep it out of gear ?

Lever first, spring, washer, nut, clip.

Yes.
Look at the clamp, one small part is not rounded, that's the part you line up to the end of the rod.
